Surgical Inflammation Alters Immune Response to Intraoperative Photodynamic Therapy

Abstract Surgical cytoreduction for patients with malignant pleural mesothelioma (MPM) is used selected as a part of multi-modality management strategy. Our group has previously described the clinical use photodynamic therapy (PDT), form non-ionizing radiation, an intraoperative option MPM. Although necessary removal bulk disease, effects surgery on residual MPM burden are not understood. In this bedside-to-bench study, Photofrin-based PDT introduced possibility achieving long-term response in murine models tumors that were surgically debulked by 60 – 90%. Thus, addition provided curative potential after incomplete resection. Despite success, we postulated surgical induction inflammation may mitigate comprehensive disease to further therapy. Utilizing validated tumor incision (TI) model, demonstrated introduction incisions had no effect acute cytotoxicity PDT. However, found induced limited generation antitumor immunity Compared alone, when TI preceded mouse tumors, splenocytes and/or CD8+ T cells from treated mice transferred less recipient animals. These results demonstrate significantly improves compared but at same time, limit generated data inform future approaches aimed blocking immunosuppression might improve outcomes combined modality treatment.
